How to Request Service

Call the Helpdesk and have the following information:

Name of template
Detail for hospital (department, base class, historical vs individual procedure, CPT/charge(s) to use)
Detail for professional (Department, POS Type, CPT(s) to use)
Letter content if different from default letter.
