HIT provides secure connections to the Health System clinical network in non-Health System buildings such as those supported by UVA ITS.

Use this form if you need a secure clinical network connection in an ITS-supported building.   A current list of UVA buildings along with IT ownership can be found here. 

Note that this form is for new VLAN connections.  If you need to move a device from one VLAN port to another in the same non-HIT supported building, please contact the HIT Help Desk at 924-5334.
