Your privacy is important to us. It’s also important that you know how we collect, use, disclose, transfer, and store your personal information through the HOOS Health Check mobile application and website.
By using HOOS Health Check, you indicate that you have read, understand and agree to this policy and that you authorize the release of information described below.
Personal information is data that can be used to identify or contact a single person.
University students, faculty and staff are asked to use HOOS Health Check every day they plan to be on Grounds, When you use HOOS Health Check, you will be asked to indicate whether or not you are free of COVID-19 symptoms. Because this attestation is linked to your mobile device and your UVA computing id and digital certificate, your response constitutes personal information.
How we use and store your personal information
We capture your personal information and store it in a secure data warehouse in the UVA Health System. We afford the same degree of confidentiality personal information captured through HOOS Health Check as is given to medical information stored by UVA Health in any other medium. We are committed to protecting the confidentiality of your personal information. We limit workforce members’ access and the ability to enter or view information based upon an individual’s role and responsibilities. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information.
As a HOOS Health Check user, your role in maintaining the security of your personal information is the same as protecting your other data at the University, including changing passwords on a regular basis and keeping them confidential. For more information, Information Security Basics.
How we use your personal information
We use your digital certificate and computing id to verify identity and eligibility to use HOOS Health Check, to assist with identification of users, and to determine appropriate services.
We use your personal information to provide you with instructions about being on Grounds, to advise you to contact Student Health or Employee Health, and to inform healthcare practitioners in those offices that you may contact to report your symptoms.
We use personal information for internal purposes such as compliance, auditing, and data analysis.
If you test positive for COVID-19, we may provide your personal information confidentially to an authorized University official responsible for student or employee attestation compliance.
We may use personal information for research. Any proposed medically related research will be conducted by UVA faculty or staff and will be subject to review and approval by UVA’s Institutional Review Board for Health Sciences Research and will be subject to your additional consent.
We collect data in a form that does not, on its own, permit direct association with any specific individual. We may collect, use, transfer, and disclose non-personal information for any purpose. For instance, University leadership will receive summary, anonymized reports showing (1) how many employees and students used HOOS Health Check on a given day and how many reported having symptoms or (2) location data for the purpose of tracking any “hot spots” of symptom reporting.
Cookies and Other Technologies
The HOOS Health Check websites may use "cookies" and other technologies such as pixel tags and web beacons. These technologies help us better understand user behavior, tell us which parts of our websites people have visited. Disclosure to Third Parties
We may provide your personal information to third parties where required by law, to be handled in accordance with relevant laws. Also, subject to a confidentiality agreement, we may grant a technology services partner with access to your personal information for the purposes of improving HOOS Health Check performance and functionality. We will not otherwise share your personal information with third parties.
Integrity and Retention of Personal Information
We will retain your personal information for the period necessary to fulfill the purposes outlined in this policy. When assessing these periods we carefully examine our need to collect personal information at all and, if we establish a relevant need, we only retain it for the shortest possible period to realize the purpose of collection unless a longer retention period is required by law.
Third‑Party Sites and Services
HOOS Health Check contains links to third-party websites, products, and services. Information collected by those third parties is governed by their privacy practices. We encourage you to learn about the privacy practices of those third parties.
We may update this policy from time to time. When we change the policy in a material way, a notice will be posted on our FAQ website along with the updated policy.
